To become a member of ANZASW print, sign and return the three declarations below. Completed forms must be posted to us, so that we have original documents on file click here for contact details.

For those that have had a police check in the previous 3 years with their current employer, please also attach the form below.  This will allow us to send you the competency material on receipt of your application:

I hereby apply for Full*/Provisional/Non-practising (circle one) membership of the Aotearoa New Zealand Association of Social Workers Inc. (ANZASW). Should my application for membership be successful I agree to abide by the Constitution and Standing Orders of ANZASW (as amended from time to time) and to pay the annual subscription and any other fees set by the Association. Furthermore, I agree to abide by the Association's Code of Ethics and I understand that I am subject to the Association's complaints, disputes and disciplinary procedures. I understand that Full membership of ANZASW is dependent upon my successful completion of a competency assessment and the award of the Certificate of Competency. I agree to notify the Association during the term of my membership, if I am convicted of any offence, am granted diversion, or am the subject of any complaint action in respect of my practice as a social worker.

I have completed and posted in addition to this application the "Form of Declaration" duly declared before an authorised witness, and both "Consent to Disclosure of Information" forms duly signed. Please note that the Association reserves the right to obtain a copy of any records of convictions held by the Police or any other government department for any member or applicant for membership of ANZASW that are not protected by the Criminal Records (Clean Slate) Act 2004, and the signed application confirms the Association's authorisation to do so.
